Well in reality a .22 and .223 bullet are pretty close in diameter. The biggest difference between the two is the velocity and weight. Because .223 has so much more velocity the wound "channel" or cavity is significantly larger and what ever you hit also feels the effects of "hydrostatic shock".

P.S. I cant focus on what im writing because I have really bad tunnel vision right now.

Summed up hydrostatic shock basically overloads your nervous system and your brain shuts down. If you want a better explanation look it up on wikipedia.
